Once a referral is made, the SLP will decide to either do a screening or move forward with an initial evaluation. First, call the parents and discuss the concerns and explain the evaluation process. Complete the Scope of Assessment form. Document on status record.

Send home a Consent for Evaluation. and PWN (prior written notice) for evaluation. There will be no meeting arrangement portion yet. Include a copy of the Health History Form for the parents to fill out and return. Include a copy of the Parental Safeguards. If the family's native language is something other than English, include a copy in that language. Send these documents to the parent via email or in the child's backpack.Document on the status record.

Once the Consent for Evaluation is returned you may begin your evaluation. Notify the nurse and teacher so that the nurse can prepare the health evaluation and the general education teacher can provide teacher input. Send the nurse a copy of the completed Health History Form.

Schedule a meeting with the LEA, general education teacher, and parent to review the evaluation results and possibly hold an IEP. Create a calendar invite and add the required team members. Generate a PWN and Meeting Arrangements (#1) for the MDT and possible IEP. Send home via email or in the child's backpack. Document on the status record.

Complete formal and informal assessments. Obtain parent input, teacher input, and complete observations in at least two settings (testing environment and classroom/specials). Follow the assessment template for Speech Only evaluations. Write your report. Complete the IEP if moving forward with an eligibility for SLI. Prepare the Eligibility Statement and Consent for Initial Provision of Services in IC.

Send home PWN and Meeting Arrangements #2. Document on the status record. Make sure at least one of the PWNs/Meeting Arrangements is signed by the parent!

On the day of the scheduled meeting, all participants should gather in the designated conferencing area or attend virtually. Start by introducing the team. Hold the MDT and review the report. Have all team members sign the report. Review the Eligibility Statement and have the parent and general education teacher sign and check Yes or No for if they agree with the eligibility. IF the student qualifies, review the Consent for Initial Provision of Services. Once consent is given, review the Initial IEP. Document the meeting on the status record.

If the meeting is virtual, send out the final documents (report, eligibility, consent for initial provision, and IEP) for digital signatures via Adobe Signature (or a similar program) or obtain signatures during the meeting if it is in-person. Document on the status record.

Send a final copy of the Report , Eligibility, and IEP with a PWN (Intent to Implement) to the parent via email or in the child's backpack. File all consents, prior notices, meeting arrangements, the signed IEP, the intent to implement, and the status record in the Confidential Folder.

Update your caseload data book and data record by adding the new student to your caseload. Begin services as soon as possible.
